Prekindergarten and Kindergarten
Note: When applying for prekindergarten or kindergarten, please complete both an application form (above) and an elementary registration form (below).

How to Register
- Fill in all the relevant information on the form either digitally or by hand.
Note: You will be required to sign the form and show legal documentation to verify the student's birth date during your next school visit.
- Save the completed form to your computer or scan/photograph the form if it was completed by hand.
- Submit the form by email or in-person.
- Email the form or scanned images as an attachment to
mayfairschools@spsd.sk.ca. You will receive a confirmation email that the registration form was received.
- If delivering the form in-person, please call the school in advance to make an appointment and sanitize your hands before entering the school.
Once the school has received your form, administration staff will contact your family to complete any remaining steps in the process and to provide additional information as required.
